Coroner’s Media Report


According to the Revised Code of Washington (RCW 68.50.300) the Coroner may release information about a person’s death to the media and general public when such information is necessary to identify the deceased, or when such information is related to an incident that is of public interest. The Coroner will make every effort to first notify the family of the deceased before information is released to the public.

June 28, 2021

The Skagit County Coroner’s Office has completed the examination of a 58 year old male named Gregory Mueller of Seattle, Washington who was involved in an accident during a sailboat racing event on June 22, 2021 along Pudget Sound north of Guemes Island. The cause of death has been determined as salt water drowning and the manner of death is accident. No further information will be released.

February 16, 2021

The Skagit County Coroner’s Office has completed the examination of a 32 year old female named Kamran Cohee who died on 2/13/2021 in Big Lake, WA. The cause of death has been determined as gunshot wound of head and the manner is listed as homicide. No further information will be released from this office.

October 5, 2020

The Skagit County Coroner’s Office has completed the examination of a 9 year old boy named Timmothy Barela who died at a local hospital after being found during a residential fire near Lake Campbell, Anacortes, WA on 9/29/2020. The cause and manner of death are pending at this time. An update will be provided when available.

September 22, 2020

For information regarding the death of Timothy Crews who was found deceased at his home on May 5, 2020, please refer to the Skagit County Prosecuting Attorney’s Office or the Skagit County Sheriff’s Office. No further information will be released by this office.

September 16, 2020

The Skagit County Coroner’s Office has identified human remains found along the Skagit River near Rockport on Sunday September 2, 2020 as 10-year-old Sage Adams who went missing on July 7, 2020. The cause and manner of death have not been determined at this time. An update will be provided when further information is available.

August 10, 2020

The Skagit County Coroner’s Office has completed the examination of a 76 year old male named Raymond S. Koladycz who was involved in a motor vehicle collision with another vehicle on Highway 20 and Slough Bridge in Mount Vernon, WA on August 5, 2020. The manner of death has been determined as an accident with the cause of death as multiple blunt force injuries sustained from the motor vehicle collision.  No further information will be released at this time.

The Skagit County Coroner’s Office has completed the examination of a 62 year old female named Jill L. Powell who was involved in a motor vehicle collision with another vehicle on Highway 20 and Slough Bridge in Mount Vernon, WA on August 5, 2020. The manner of death has been determined as an accident with the cause of death as multiple blunt force injuries sustained from the motor vehicle collision.  No further information will be released at this time.

The Skagit County Coroner’s Office has completed the examination of a 56 year old male named Andrew W. Young who was involved in a tractor accident on a farm in Sedro Woolley, WA on the morning of 7/29/2020.  The cause of death has been determined as mechanical asphyxia and the manner of death is ruled an accident. No further information will be released at this time.

June 3, 2020

The Skagit County Coroner’s Office has confirmed the identification of a woman involved in a fatality on I-5 near the Blackburn Overpass in Mount Vernon, Washington on the morning of 5/26/2020. The woman has been identified as Leticia Maldonado-Reyes, age 57 of Mount Vernon, Washington. The cause of death is determined to be multiple blunt force injuries and the manner of death has been ruled a suicide. No further information will be released by this office.

December 30, 2019

The Skagit County Coroner’s Office has completed the examination of a 36 year old male named Christopher M. Brock. On December 27, 2019,  Christopher Brock was involved in a motor vehicle vs. train collision at Garrett RD and Highway 20, Burlington, Washington. The cause of death is multiple blunt force injuries and the manner is accident. No further information will be released at this time.

December 19, 2019 

The Skagit County Coroner’s Office has completed the examination of a 62 year old female, Maria Magdalena Torres of Mount Vernon, Washington.  On 12/18/2019, Maria Torres was a pedestrian who was crossing the roadway and was struck by a motor vehicle on Burlington Blvd, Burlington, Washington. The cause of death is multiple blunt force trauma injuries. The manner of death is accident. No further information will be released by this office.

December 11, 2019 

The Skagit County Coroner’s Office has completed the examination of 23 year old Douglas J. Gunter of La Connor, Washington who died in Burlington,  Washington on December  6, 2019. Douglas J. Gunter died from multiple gunshot wounds and the manner of death is homicide. No further information will be released.

December 4, 2019 

The Skagit County Coroner’s Office has completed the examination of a 53 year old male named Richard Lee Johnsen of Mount Vernon, Washington. Richard Lee Johnsen died in a residential fire on 11/28/2019, at 2250 Old Hwy 99 S., Mount Vernon, Washington. The cause of death is listed as inhalation of products of combustion due to residential fire. The manner of death is accident. No further information will be released at this time.

December 4, 2019 

The Skagit County Coroner’s Office has completed the examination of a 48 year old male, Todd E. Williams of Burlington, Washington.  On 11/28/2019, Todd Williams was a pedestrian who was struck by a motor vehicle on State Route 20 at Alta Vista Road, Sedro Woolley, Washington. The cause of death is multiple blunt force trauma injuries. The manner of death is accident. No further information will be released by this office.

December 4, 2019 

The Skagit County Coroner’s Office has completed the examination of a 53 year old female, Tonya L. Pearson of Burlington, Washington.  On 11/20/2019, Tonya Pearson was a pedestrian who was struck by a motor vehicle on I-5 south bound, mile post 207, Bow, Washington. The cause of death is multiple blunt force trauma injuries. The manner of death is accident. No further information will be released by this office.

October 22, 2019 

The Skagit County Coroner’s Office has completed the death examination of a 69 year old male named Richard Lee Taylor. On October 20, 2019,  Richard Lee Taylor was involved in a motor vehicle collision on I-5 at milepost 232 outside of Burlington, Washington. The cause of death is determined to be multiple blunt force injuries and the manner is accident. No further information will be released at this time.

May 2, 2019 

The Skagit County Coroner’s Office completed the investigation of 27 year old Michael James Schuchart.  On January 15, 2019, Michael Schuchart was a pedestrian who was struck by a motor vehicle on I-5 Southbound, at milepost 229.5, Burlington, Washington. The cause of death is determined to be multiple blunt force injuries and the manner is accident. No further information will be released by this office.

May 2, 2019

The Skagit County Coroner’s Office has identified  a male who was found deceased in the Skagit River at 301 West Division Street, Mount Vernon, Washington. The decedent is identified as 37 year old Adrian S. Westhof of Mount Vernon, Washington. The cause of death and manner of death are pending at this time. An update will be provided when available.  

May 2, 2019

The Skagit County Coroner’s Office has identified  a male who died on 04/30/2019 after the vehicle he was driving was struck by another motor vehicle on I-5 near exit 221, Conway, Washington.  The decedent is identified as 33 year old Curtis Miller of Sedro Woolley, Washington. The cause and manner of death are pending at this time. An update will provided when available.

The Skagit County Coroner’s Office has completed the examination of a 68 year old male identified as Dwight L. Woodruff who died in a residential house fire on February 14, 2019 in Anacortes, WA. The cause and manner of death has been determined as an accident due to smoke inhalation. No further information will be released at this time.

January 28, 2019

The Skagit County Coroner’s Office has identified  a male who died on 1/27/2019 after he was struck by a motor vehicle on I-5 near milepost 237, Bow, Washington. The decedent was identified as 45 year Oly S. Oberg. The cause of death has been determined as multiple blunt force injuries. The manner of death has been ruled an accident. No further information will be released by this office.

January 17, 2019

The Skagit County Coroner’s Office has identified a male who died on 01/15/2019 after he was struck by a motor vehicle on I-5, mile post 229.5, Burlington, Washington. The decedent is identified as 27 year old Michael James Schuchart of Burlington. The cause and manner of death are pending further investigation at this time. More information will provided on a later date.

The Skagit County Coroner’s Office has completed the examination of a 19 year old male named Dante J. Taylor who died of injuries related to a train accident that occurred in the early morning hours on January 14, 2019. The manner has been ruled as undetermined.

August 28, 2018

The Skagit County Coroner’s Office has completed the examination of a 27 year old female identified as Vanessa A. Cons who was found deceased inside a home in Mount Vernon, WA on the afternoon of 8/26/2018. The cause of death has been determined as multiple sharp force injuries from being stabbed by another. The manner of death has been determined to be a homicide. No further information will be released by this office.

August 13, 2018

The Skagit County Coroner’s Office has completed the examination of a 22 year old male named Chase Alexander Blair who was involved in motor vehicle collision on I-5 at mile post 232 in Burlington, Washington on 08/09/2018. The cause of death is multiple blunt force injuries to the head. The manner is accident. No further information will be released at this time.

August 1, 2018

The Skagit County Coroner’s Office has completed the examination of a 52 year old female named Janette Newman who was involved in a car fire on 7/16/2018 on Capehorn Road in Sedro Woolley. The cause and manner of death has been determined as an accidental death due extensive burns and smoke inhalation from the car fire. No further information will be released at this time..

The Skagit County Coroner’s Office has completed the examination of a 64 year old female named Cynthia Wiegand Prosser who was involved in motor vehicle collision on I-5 at mile post 221 in Conway, Washington on 7/18/2018. The cause and manner of death has been determined as an accidental death due to multiple blunt force injuries to the head, chest, and abdomen. No further information will be released at this time.

The Skagit County Coroner’s Office has completed the examinations of two individuals named John Gore age 74 and Bernard Parker age 65 who were involved in a single motor vehicle collision on 7/24/18 on Baker Lake Road. The manner of death has been determined as an accident with the cause of death as multiple blunt force injuries sustained from the motor vehicle collision. No further information will be released at this time.

July 9, 2018

The Skagit County Coroner’s Office has completed the examinations of two pedestrians who were struck at the intersection of Rhodes Rd and SR 20 in Sedro Woolley in the early morning hours on 7/8/2018. The victims involved were identified as Patricia Gunsolley, age 67 and Wayne Gunsolley, age 64. The cause and manner death for both individuals was determined to be an accident due to multiple blunt force injuries. No further information will be released at this time.

May 30, 2018

The Skagit County Coroner’s Office has determined the cause and manner of death of a 30 year old, Nicole K. Barney who died in Anacortes, Washington on January 25, 2018. The cause of death is saltwater drowning and the manner is accident.

April 18, 2018

The Skagit County Coroner’s Office has completed the identification and examination of two individuals involved in a motor vehicle collision on Little Mountain Road on the evening of 4/16/2018. The driver was identified as Michele N. Kadyk age 48 and the passenger was identified as Trevor J. Clifford age 38. The manner of death has been determined an accident and the cause of death for both individual’s was multiple blunt force injuries due to the motor vehicle collision. No further details will be released by this office.

April 12, 2018

The Skagit County Coroner’s Office has identified human remains found east of Cascade River Park near Marblemount, Washington. The deceased is identified as Edgar James Robinnette, born in 1981. The remains are undergoing analysis. The cause and manner of death are pending. More information will be provided as it is available.

The Skagit County Coroner’s Office has completed the identification and examination of 46 year old Michelle R. Patterson.  The cause and manner of death is pending further investigation. More information will be provided as it is available.

The Skagit County Coroner’s Office has identified the individual who died in Mount Vernon, Washington on January 23, 2018 in a house fire.  She has been identified as 55 year old Renee L. Burkhead- Gardner. The cause of death is conflagration. The manner of death has been ruled an accident.

The Skagit County Coroner’s Office has completed the identification and examination of 63 year old Garry W. Regan.  Mr. Regan’s remains were discovered on 03/10/2018.  The cause of death is multiple blunt force injuries and the manner of death is  suicide.

The Skagit County Coroner’s Office has identified the remains of 41 year old Jason Lee Pietrzykowski.  Mr. Pietryzkowski’s remains were discovered on 03/03/2018 by a climber on the slope of Mt. Erie.  The cause and manner of death are pending.

The Skagit County Coroner’s Office has completed the examination on an unidentified female who died in Mount Vernon, Washington on January 23, 2018. The cause of death is conflagration. The manner of death has been ruled an accident. An update will be provided when identification is established.

The Skagit County Coroner’s Office has completed the examination of a 36 year old male named Allen L. Vike Jr. who died in Concrete, WA on December 18, 2017 after sustaining a self-inflicted gunshot wound to the head. The manner of death has been ruled a suicide.

Zapata who died in Mount Vernon, WA on December 17, 2017 after sustaining a gunshot wound to the head. The manner of death has been ruled a homicide. The Skagit county Coroner's office had completed the examination of a 61 year old male named Basilio Cuellar who died on November 12, 2017 in Mount Vernon as a result of multiple blunt force injuries sustained from a motor vehicle collision. The manner of death is accident.

The Skagit County Coroner’s Office has completed the examination of a 54 year old male named Terry D. Rose who was found along Riverbend Road in Mount Vernon, WA on November 8th, 2017. The cause and manner of death are pending but there is no indication of foul play

The Skagit County Coroner’s Office has completed the investigation of death of two children involved in a house fire on Bay View Road On July 15, 2017: Xavier R. Strawn, age 6, and Rose R. Mihalovits, age 9. The manner of death has been determined as a homicide and the cause of death was due to carbon monoxide toxicity and heat effects from a house fire. No further information will be released.

The Skagit County Coroner’s Office has completed the investigation involving the death of a 11 year old female named Araceli Dominguez Sebastian who died on July 8, 2017. The determined Manner of death is Accident and the Cause of Death is Carbon Monoxide Toxicity

The Skagit County Coroner’s Office is investigating the deaths of two children who died in a house fire on Bayview Rd. , Mount Vernon, Washington. The examinations will be performed later this week and the identifications will be released once scientific I.D. is established.

The Skagit County Coroner’s Office has completed the examination of an 11 year old female who died at 17000 Block of Cook Rd., Burlington, Washington. The cause and manner of death are pending toxicological analysis and further studies. Information will be updated when the investigation is completed.

The Skagit County Coroner’s Office has completed the examination on a 24 year old female, Shannon Denise Cruz, who died on March 28, 2017. The manner of death has been determined to be homicide with cause of death of hemoperitoneum; due to transected liver; due to blunt abdominal trauma.

The Skagit County Coroner’s Office has completed the investigation of the death of Paul Marvin Burnham found on 1/25/2017. The manner of death has been determined to be natural with cause of death of congestive heart failure secondary to coronary atherosclerosis, due to hypertensive cardiovascular disease.

The Skagit County Coroner’s Office has completed the investigation of Boback Wheeler Bahandori who died in the Skagit County Jail on 12/20/2016. The manner of death has determined to be natural with the cause of death as septic shock due to staphylococcal aureus bacteremia from an arm abscess due to intravenous drug usage.

The Skagit County Coroner’s Office has completed the identification and examination of an 83 year old male named Paul M. Burnham of Anacortes, WA. The cause and manner of death are pending further investigation, however, there was no trauma noted during the examination.
